Workshop dates: January13th, February 10th, March 10th, April 14th, May 12th and June 9th.

Each workshop is different! You may register for as many as you'd like!

 

Cookshop for families is a series of six, free monthly workshops for parent and guardians. Its the adult version of the Cookshop classroom program being taught to students. At each workshop parents will prepare a recipe, learn nutritional tips and more. Each month, participants will receive the ingredients to cook simple and delicious recipes at home with your families.

Registration Requirements

Proof of Address: Two proofs of address must be provided. Current copies of the following documents may be used as proof of address. Any two of the documents below will be accepted as proofs.

*Gas or electric bill in parent's or guardian's name

*Document or letter from a federal, state or local government agency, including the IRS, Housing Authority, Human Resources Administration, Administration for Children's Services, and US Citizenship and Immigration Services.

*An original lease agreement, deed or mortgage statement for the residence

*A property tax bill for the residence

*A water bill for the residence

*Official payroll documents issued within the last 60 days. A letter form an employer is not acceptable.

 

Proof of Age: Original Birth Certificate, Baptismal Certificate, Passport or Immigration Record.

Proof of Immunization: 3DPT, 3 Polio, 2 MMR, 3 Hepatitis B and 1 Varicella

The child you are registering MUST come with you.

Parent Survey & Program Selection Form

 

Parent of children who have been identified as English Language Learners must select a program for their child. The Parent Survey and Program Selection Form provides parents and families with the option of choosing the most appropriate program model for their child. Please note that at this time PS85Q only offers the Freestanding English as a Second Language Program (ESL). Parents do have the option of choosing a different program at another school that offers the program they have selected. Parents are responsible for arranging transportation if they choose a program in a different school. The Department of Education will not provide transportation.
