If you lost or misplaced the survey you should call the

Survey Hotline at 1-800-690-8603

The survey period ends April 12, 2013 but we encourage parents to fill it out as soon as possible.

Please fill out the survey and place it in the envelope provided.

Please write down your child or children’s names on the envelope so we can give a raffle ticket to your child(ren) to be included in our raffle (don’t worry, the survey is still anonymous; the envelopes are thrown out and the survey sheets are scanned

You may also fill out the survey online. This option helps the school obtain a green badge!

If you do the survey online, make sure to print out a confirmation and send it to Ms. Arcos, so your child can get a raffle ticket.
